We will ask you a few preliminary questions about your situation and make an appointment to visit the applicant.
Step 2. The Interview
We will arrange to meet the applicant to better understand the current health status and determine qualification for admission according to the regulations that govern our operations.
Physician Recommendation: Admission begins with a recommendation from the applicant's physician. This allows us to clearly understand the health status of the individual and all of the details of care that will be needed. This is also required by the laws and regulations that govern our facility in the State of Wisconsin.
Step 3. The Preliminary Admission
This meeting will handle the business of admission for the qualified applicant. We will review the physician's referral documents and begin to process the application. This involves completing financial papers, reviewing the terms of residence, and reviewing all of the details of care giving for the applicant.
Step 4. The Residential Agreement
With all of the facts in hand, we will prepare the Residential Agreement for approval by signature of the applicant and/or the family members involved.
Step 5. Moving In
New residents are encouraged to bring some of their own furniture. This usually includes the bed, dresser and a night table plus additional chairs and tables appropriate for the space in the unit selected. If this is not possible, we can make temporary arrangements for all of the furnishings required. We will help to place and put away all of the resident's belongings and introduce our entire staff. At some point during the first day we will take a photograph of the resident for our care file.
Once settled, we will sit down for a talk about preferences for meals and activities. We want to know all we can about our resident's living habits and preferences. Our goal is to receive the new resident warmly with a genuine interest in who they are and what they prefer.
